to save the girl がattemptを修飾する不定詞の形容詞的用法です
直訳「ケイトの、その少女を救う試みは」

cost 動詞
cost-cost-cost なので過去形も過去分詞も同じ形
cost O1 O2で、「O1にO2を払わせる、O1にO2（という損失）をもたらす」みたいな意味なので
cost her her life は「ケイトに、ケイトの命の損失をもたらす」
herはケイトですね。
