まで見たときに、強調構文の可能性を考えられたかどうかがポイントです。

強調構文は、It is / 関係代名詞　を抜かしても普通に文として成立します。（isは時制によって変化します）
そもそも無くても文として成立するんですが、強調のために、It is / 関係代名詞を無理やり入れたものなのです。

パターンとしては2つで
a)It is [名詞のまとまり] +関係代名詞 + 不完全文
b)It is [副詞のまとまり] + that + 完全文
正確にいうとa)は関係代名詞と言って良いのか微妙なんですが。
またb)のときにwhereとかwhenなど関係副詞を使うのも無くはないという説明もありますが、とりあえず受験とかでは出てこないと思います。

今回はa)のパターンです。
It was this hospital which I visited last month.
whichのうしろは、visited の目的語が抜けた不完全文です。
It was とwhichを抜かすと
I visited this hospital last month.
この文の、this hospitalを強調するために
It was / whichをぶち込んだと理解します。

選択肢①と選択肢③はthatの後ろが完全文になっています。
ですから例えば
It was [at/in this hospital] that I was born.
という感じで、強調する部分が副詞句なら①③は正解になります。

④はwhereの後ろが不完全文だし、もし仮にwhereを使うなら（受験では…と先程言いましたが）、その前の強調したい部分が副詞のまとまりじゃないといけないんです。
