害は子供に与えられましたか？
→何が害を子供に与えましたか？
→子供は害を受けましたか？

この文を能動態にする場合、主語は自分で考える必要があります。

例えば、
That apple was eaten.
を能動態にする場合、
Someone ate that apple.
になります。

よって、
Something did harm to the children.
を疑問文にして
Did something harm to the children?
ではないでしょうか？
