1〜3は受動態の形に、4〜5は括弧内の語句を使って英語に直して欲しいです。
出来ればそうなる理由も付けていただければありがたいです。

と私は解答しました。どこが違うのかの説明も欲しいです。

受け身の文の鉄則
元の文（能動態）の目的語を、受け身の文の主語とする

1. What was invented by him ?
「彼によって」by him

2. 「ケーキは誰によって食べられたのですか。」の意味にします。
①By whom was the cake eaten ?
②Whom was the cake eaten by ?
③ Who was the cake eaten by ?
いずれも正解です。
☆Who were eaten 〜だと、誰が食べられたのですか？の意味になってしまいますし、Whoは３人称単数扱いなので、wereも間違いです！

3.hear ofを群動詞と捉えて、群動詞の目的語himをHeとして受け身の文の主語にします。
He has never been heard of by us since then.

4. I was spoken to by a foreigner on the street.
speak toを群動詞とみなします。

5. Many machines are being made in China today.
